Home nursing

As a Medical Centre (Medisch Huis) we ensure the continuity of nursing care for our enrolled patients — including at home. Home nursing is coordinated within the practice by Annick Thoen.

For patients

Do you need home nursing care?

• Care always starts via our coordinator. Annick assesses your care needs together with you and your doctor and makes sure the right care is started in time.
• Home nursing within the Medical Centre is intended for patients who are enrolled with us and is prescribed by a doctor.
• Get in touch via the e-mail address above, or raise it during your consultation.

Our preferred partner for home nursing is EBS Thuisverpleging, who are active on the Left Bank (Linkeroever) among other areas. In consultation with our coordinator, they provide the care at your home.

For home nurses

Are you a self-employed nurse or nursing organisation who would like to collaborate with our Medical Centre?

• For our enrolled patients, the start and coordination of care go through our coordinator.
• Prescribed nursing care for enrolled patients is invoiced according to the nomenclature.
